Universal Primary Care Care Management Department

Universal Primary Care (UPC) is a Federally Qualified Health Center dedicated to whole patient care.  It is our goal to ensure that every patient receives the care they need regardless of income.  At UPC this care goes beyond the physician’s office.  UPC staffs two RN Care Managers who assist individuals with self-management and patient education and two Care Coordinators who assist patients with barriers to care and provide access to public programming.

Insurance Enrollment Assistance – NYS of Health Certified Application Counselors

(This includes Medicaid enrollment)

 

Open Enrollment begins November 1, 2016

Prenatal Smoking Cessation – Baby and Me Tobacco Free

Breast Feeding Education – Certified Breastfeeding Consultant

Pregnancy Education and Child Birth Classes

Diabetes Education

Transportation Assistance

Falls Prevention – Healthy Community Alliance

Gym Referral – YMCA Rx Referral
