Universal Primary Care (UPC) is a Federally Qualified Health Center dedicated to whole patient care. It is our goal to ensure that every patient receives the care they need regardless of income. At UPC this care goes beyond the physician’s office. UPC staffs two RN Care Managers who assist individuals with self-management and patient education and two Care Coordinators who assist patients with barriers to care and provide access to public programing.
